Your Next Steps: How to Proceed With Your Credentialing at Amplifon
Option #1: You Have a CAQH Profile
If you’re already part of the system, you likely have a unique identifier. (If you are unsure, see What is a CAQH Number below).
- Access the CAQH provider login portal.
- Make sure your profile data is accurate and up to date.
- Make sure you have a current attestation. Profiles must be certified as accurate every 120 days.
- Grant Amplifon authorization to view your data.
- Fill out our online request form.
- Send your CAQH Profile ID to credentialing@amplifon.com so we can start processing your file.
Tip: Regularly maintaining your profile prevents credentialing delays. A quick CAQH provider log-in throughout the year can ensure your status never lapses.
Option #2: You Don’t Have a Profile Yet
New to the system? You’ll need to create an account and build your profile.
- Go to the ProView registration page.
- Complete the registration to receive your CAQH Provider ID.
- Complete the data entry for your professional profile.
- Upload required documents (license, insurance, etc.).
- Attest that everything you’ve provided is accurate
- Once you’re done, send your CAQH ID (located at the top right-hand side of the screen) to credentialing@amplifon.com so we can start processing your file.

CAQH ProView is the healthcare industry’s resource for self-reporting professional and practice data. Instead of completing a separate, manual application for Amplifon, we simply access the data you have already entered into this universal provider platform.
It serves as an easy one-time data entry point. Once your profile is built, you grant Amplifon access, and we handle the rest!
Good hearing matters
